My supervisor said that it is beneficial to be specific in what to observe in assessments as we, in acute setting, may have limited time. Now, I shall started off with “What to assess” in an initial assessment of a cerebellar haemorrhagic stroke, which I was exposed to several of them for the past week.
This list excludes obtaining social history and cognitive status of the client.
Physical Assessment of Cerebellar Stroke
- Any double vision?
- Any giddiness?
- Strength and AROM of UL and LL
- Finger-nose test of both UL
- Pronation-Supination test in both UL
- Coordination test in LL
- Truncal ataxia test in sitting position
- Observe sit to stand
- Observe LL coordination when marching on the spot
- If possible, walk a few steps (note any wide-based gait etc.)
Cerebellar stroke clients will usually complain of giddiness due to double vision. Hence, it will be wise to prepare a plastic bag and tissue lest they feel like vomitting. Another point to note about cerebellar stroke clients is that they usually have minor deficits in their strength and AROM of both UL and LL.
