I was reading through one of the OT blogs in my Links and I found a useful site which the author have found.

The OT Plan is really useful for OTstudents and practitioners alike to get more ideas on treatment planning and modalities. It’s simple! Just click on the area you want to tackle. Click on the treatment materials you want to use. And there you have it - treatment plan/activities which details the purpose, materials needed, process, ratings, and comments.
